Report on Changes in Educational Locations or Instructional Sites

If you have established any new educational locations or instructional sites at which IACBE-accredited programs are offered, or have discontinued/closed operations at any location or site, please list the locations/sites in the following form, and identify the IACBE-accredited programs that are, or were, offered at those locations/sites. In addition, if the programs are, or were, delivered in partnership with other institutions at those locations/sites, please identify those institutions as well.

Space is provided in the form for three locations/sites. If you have established and/or discontinued more than three educational locations or instructional sites, please submit an additional reporting form.
